Referring Someone to Hospice

Anyone can call hospice. The patient, patient’s physician, a family member or a friend may initiate hospice services by calling our office. Once a referral has been made, our Admissions Coordinator schedules a visit to meet the patient and family and answer questions. Following this visit, a hospice nurse and social worker meet with the patient and family to assess their needs and develop a plan to meet those needs.

Hospice care is appropriate for those persons who have a life-limiting illness with an approximate life expectancy of six months or less. Hospice patients agree to accept treatments that focus on symptom management rather than cure. All patients remain under the care of an attending physician who gives consent to hospice services.
