Good Sam works alongside a patient’s primary care physician. The physician is a member of the care team and remains an active participant in the direction of treatment. As a patient progresses, Medicare recognizes that it can be medically difficult to determine when a patient is eligible for hospice, meaning prognosis is six months or less if the terminal disease runs its usual course. The potential for a patient to improve under hospice care is not a contraindication for hospice. Approximately 10-15% of hospice patients are eventually discharged because of improvement, often due to the expert care that hospice provides. Good Sam offers a free assessment by a hospice nurse to assist with determining whether a patient is eligible for hospice care. In addition, Good Sam offers an online test (Hospice Eligibility Test) to guide with this next step.
